Care coordinators help address the physical, behavioral health, and social services needs and preferences of each individual served by our program. Whether services are provided directly by the Center for Holistic Health & Wellness or through collaboration with medical and other service providers in the community, care coordinators work to ensure seamless transitions for individuals across the full spectrum of health services.
